The Chronic Disease Management Journey

Introduction and Intake
Your members/employees will meet with their Dedicated Case Nurse and Doctor. A Medical Administrator will coordinate a personalised Disease Management Programme. Together, they will monitor your members/employees progress and answer their medical queries.
Evaluation and Consultation
Your members/employees dedicated Case Team will consult on their behalf with leading and multidisciplinary specialists to ensure the right diagnosis is achieved and best possible treatment is implemented. If needed, they will refer your members/employees and coordinate additional medical tests.
Recommendations and Planning
Based on the specialists’ consolidated opinions, the Case Team will share and explain the treatment options available and create a Disease Management Programme for your members/employees. The Team will share and discuss this plan with their treating doctor supporting their decision-making process.
Ongoing Support
Upon need, the Team will refer your members/employees to local and global specialists and will coordinate their care. Medix will guide and support your members/employees through the implementation of their Chronic Disease Management Plan and will provide them with the necessary tools and information to cope with their condition.
